PARTICIPANT AGREEMENT & DATA CONTRIBUTION CONTRACT

1. PARTIES

This Participant Agreement (“Agreement”) is entered into between:

Joy 4 Healthy Living™️ Powered by WatsonGuard™️

(“Provider”)

and

Participant: You are the Participant

(“Participant”)

2. PURPOSE OF AGREEMENT

The purpose of this Agreement is to establish participation in a structured wellness monitoring and data collection program utilizing the WatsonGuard™️ system.

The Participant agrees to provide consistent daily health inputs in exchange for access to wellness monitoring, structured guidance, and participation in ongoing system development, research, and patent-supported methodologies.

3. PROGRAM OVERVIEW

Participant will engage in a structured daily monitoring program that includes, but is not limited to:

  – Blood pressure monitoring

  – Heart rate tracking

  – Hydration logging

  – Nutrition and sodium intake tracking

  – Medication and supplement tracking

  – Activity and symptom reporting

  – Lifestyle and behavioral inputs daily

All data contributes to the WatsonGuard™️ monitoring system, including but not limited to the WatsonGuard™️ 6-Point Safety Model.

4. PARTICIPANT RESPONSIBILITIES (MANDATORY COMPLIANCE)

Participant agrees to:

1. Submit daily health inputs as required by the system

2. Provide accurate and truthful information

3. Follow all program guidelines and instructions

4. Maintain consistent participation without interruption

5. Respond to follow-up requests when necessary

Daily participation is REQUIRED.

Failure to submit required inputs will be considered non-compliance.

5. DATA USAGE & INTELLECTUAL PROPERTY

Participant acknowledges and agrees:

  – All submitted data may be used for:

  – System development

  – Clinical pattern analysis

  – Patent documentation and filings with the United States Patent and Trademark Office

  – Internal research and improvement of WatsonGuard™️

  – Data will be handled in accordance with applicable privacy standards

  – Personally identifiable information will not be publicly disclosed without consent

Participant grants Provider a non-exclusive, perpetual, royalty-free right to use anonymized data for research, development, and intellectual property purposes.

6. COMPENSATION / PROGRAM STRUCTURE

Participant is enrolled in a structured participation-based program.

  – In exchange for consistent daily compliance, Participant receives:

  – Access to WatsonGuard™️ monitoring

  – Personalized wellness insights

  – Structured health tracking and reporting

  – This agreement may operate as:

  – A subsidized or sponsored participation      model, OR

  – A reduced-cost program contingent upon compliance

7. NON-COMPLIANCE & TERMINATION

Strict compliance is required.

The following constitutes breach of agreement:

  – Missing required daily inputs

  – Repeated incomplete submissions

  – Providing inaccurate or misleading data

  – Failure to follow program guidelines

If Participant fails to comply:

1. Participant will receive a notice of non-compliance

2. If unresolved, participation will be terminated immediately

3. Participant will be removed from the study program

4. Participant will be transitioned to a standard paid service model, OR access will be revoked

Provider reserves the right to terminate participation at its sole discretion for non-compliance.

8. TRANSITION TO PAID SERVICES

Upon termination due to non-compliance:

Participant may:

  – Enroll in a paid WatsonGuard™️ monitoring program, OR

  – Discontinue services entirely

No continued access will be granted under the participation model once the Agreement is breached.

9. MEDICAL DISCLAIMER

This program is not a substitute for professional medical advice, diagnosis, or treatment.

Participant agrees to:

  – Consult with a licensed healthcare provider for medical concerns

  – Not rely solely on WatsonGuard™️ outputs for medical decisions

10. LIMITATION OF LIABILITY

Provider shall not be held liable for:

  – Participant’s failure to follow medical advice

  – Inaccurate data submitted by Participant

  – Outcomes resulting from non-compliance

—

11. VOLUNTARY PARTICIPATION

Participation is voluntary.

Participant may withdraw at any time; however:

  – Withdrawal results in immediate termination of access

  – No continued participation benefits will be provided

12. ACKNOWLEDGMENT

By signing below, Participant acknowledges:

  – Full understanding of daily compliance requirements

  – Agreement to provide consistent and accurate data

  – Understanding that failure to comply results in termination

  – Consent to data usage for system development and patent support
